TELEHEALTH CONSENT FORM

 Beacon House is now able to provide treatment services through Telehealth

Telehealth services will include: Individual counseling, group counseling, family/couples counseling, psychiatric evaluation, medication management, or other behavioral health services you seek.

Beacon House will send all consent forms through the client private portal KIPU EMR.
