All our Programs require a Clinical Assessment. Residential Treatment, Partial Hospitalization Program (PHP), Intensive Outpatient (IOP), General Outpatient (GOP), and Recovery Residences.
An assessment helps us understand how substance use or mental health issues have impacted and may currently be affecting your life. It also guides us in determining the appropriate level of services and most beneficial treatment interventions for you. Your Level of Care treatment is based on the clinical assessment of risk levels defined by the American Society of Addiction Medicine (ASAM).
Plan on scheduling two hours for your Assessment. Things to bring:
list of all current medications and dosages.
Insurance card
Parole officer contact information
License or Identification (if you have have one)
If you have completed an assessment within the last six months at another agency or have recently been released from an inpatient program, we may be able to transfer your assessment.
